Why does my 728x90 unit always display 468x60 ads?

So I just recently realized this was happening and is probably the cause of such poor performance since about 2 weeks ago. My 728x90 spots are always showing 468*60 ads and obviously the smaller size has less CTR and pays less. Anyone else have this issue?

If no 728x90 ads are available, a 468x60 ad may be shown because it fits in the slot.

That also goes for other compatible sizes.

Make sure the container is large enough to actually fit a 728x90 ad.

Pretty sure the container is large enough and the spot was showing 728x90 ads before.

How does DFP compare to just running adsense?

Is this based only on what you see, or have you found confirmation in your Performance Reports that fewer 728x90 ads are being shown?

What I see, also the Ctr rates and earnings are at an all time low.

Performance reports > Advanced reports > Ad sizes, click 728x90 to get a line graph.

(And then compare that with other sizes, of course.)
