adsense account disabled

Hello everyone.
My account got disabled on 26 of july a day before my payment was supposed to be released. I was using this account for nearly 5 years. Never had any warning or anything. Out of no where my account is disabled. I was making my living of adsense. I have received email just as everyone that my adserving is being disabled. But not mentioned why. Just standard template which they send to everyone. Can you guide me how can I request them to reinstate it or inform me how can I fix the issue they have problem with. Or guide me how should I go by it. I need help and looking for ideas that will actually work.
Which email ID I can reply them and send my appeal to.

Out of interest, does your site have a valid privacy policy ?

You cannot fix the issue if you do not know what the issue is. I am surprised they haven't said why it was disabled.
