Do Google expect us to apply the "no similar ads" rule to image ads.

Adsense image ads have not particular format, and have no indication that they are Google ads. How can be make sure that they cannot be confused with image ads from other sources?

I have turned off Adsense image ads because some of them I see on the site are very similar to affiliate banners I use. Am I over-reacting?

Competitive Ads and Services
In order to prevent user confusion, publishers may not display Google ads or search boxes on websites that also contain other ads or services formatted to use the same layout and colours as the Google ads or search boxes on that site. Although you may sell ads directly on your site, it is your responsibility to ensure that these ads cannot be confused with Google ads.

That's a good question. If you want to be extra sure, it seems that according to the above policy all you have to do is label the affiliate ads according to the source of the ads. Blogads and other competing services already label their ads.
