In this kind of case, I have found it is good to find a competitor, or just some other site you like, that has Adsense on it. Whenever Adsense is "down" or acting otherwise funny, the first thing to do is check this other site that has Adsense, that will tell you if it's only your site or everyone is having problems.
I do exactly the same when this happens. It was the same for about 3 or 4 sites.
I did some digging around and realized they might take it down if you don't have a privacy policy up and in place.
This is not the case for two reasons.
1: This has happened before and way before the policy existed.
2: The site has an updated policy.
In my case, however, I didn't have a privacy policy until about an hour ago when I created one.
Why ?
It's not as if you can turn round and say you didn't know. Theres been a message on the adsense page every time you logged in. And it still appears... Long after the cut off date.
Sorry I cannot answer your other questions. I only use adsense.

Can you afford to lose 100-150 clicks a per day and not worry about it ? I don't think so.
Actually I subscribe to the Google glass ceiling theory and the way they fix total earnings and eCPM levels for websites through smart pricing.. or what ever..
What I mean to say is that fall in clicks has led to increase in EPC and I have not lost any money.. I know once, I get my clicks back.. I am going to see reduced EPC and same level of earnings.. So I am waiting to check out my hypothesis.. ;-)

I've no idea. It has happened before in the past. No doubt it will happen again until Google locates the error and fixes it.
It happened more often than not with new code, which is one of the reasons why I reverted back to the old-style code, but it still sometimes happens.
This site was somehow missing a privacy policy, which has since been added. Trying to see if I have any other sites without privacy policies to see if the same is happening.
When CTR goes down for me, so does EPC thus, compounding loss of income.
BUT
Before month's end I have no doubt yet once again I'll reach that non-existent mythical glass ceiling on earnings.
Last three months were +/- <1% month to month.
Pure imaginings?
