You don't know what is a Premium Publisher?
Consensus usually has it that it's 20 million page views per month however some have been accepted with about 10 million in specialist and niche sectors and some with considerably more than 20 million hear nothing.
And I don't believe you can apply, I think you are invited...obviously I'm not one of them.
any way you answered my question because you went to that site saw it and even realized it was cnn related site and said may be they were premium users...
plus for your kind information we do have news site which do fall under your criteria of 20 adsense views or impression per month...but we were never said or sent email by google saying we are premium user and we will have those privilege...even was not informed by my employees...
so,it is not good to jump to conclusion i guess....
and one more thing i have seen lots of small sites also changing it...
does it mean that they are doing thing against TOS?

your critera of 20
It's not my criteria, these are figures which are generally accepted as being the minimum for Premium Publisher status BUT some may be accepted for less than 10 million and some refused for 40-50 million. I guess it really depends on the quality and geographical spread of the site and the possible ad inventory that Google has for that niche/country/region.

It is possible to customise the appearance of a standard ad without contravening the TOS and changing font sizes were one of those brought in a month or so ago by Google.
obviously I'm not one of them.
