Another case (separate account -- this is with the business that I work for): We had a 120 skyscraper on a page. I added a 468x60 and that worked; I added a second 468 and at first it worked, but then it quit acknowledging the second 468 ad (even though the preview tool showed that there were plenty of targeted ads that it should fit w/o problem). I then eliminated the second 468 and instead went with a 180x150 in the left corner, and that worked but it seemed to knock out the functionality of my 468 banner.
I understand that the system is new and that Google may still be working out the bugs, but I wonder if I'm alone on this, if it's something I may be doing wrong (or misunderstanding), or what the deal is here.
We are also in a category where there are supposed to be an abundance of ads, but we have many pages where only one ad format is filled and the other 2 ad formats are not shown. You may also want to check out this thread:

My earnings yesterday went to 3X what they had been on a daily average for the past month, I think in no small part because of the ads (and that's even with the problems that some of my pages had with them).
The system seems to really dislike the three ad approach, so I've backed off on that for the moment; the two-ad deal isn't perfect but seems to work better (at least for the sites where there are ample ads that appear available from the preview tool).
