Urgent! Google's AdSense 1099 info

I have misplaced my 2003 Google AdSense 1099-MISC (it *was* "MISC", wasn't it?). I have the financial info, of course, but I need to know what Payer info Google used on it.

1) What was the Payer's (Google's) EIN? According to their IPO papers, it's 77-0493581... is this what they used on their 1099?

2) What did they use as the Payer's address/phone number?

3) Did they include our AdSense account number in the "Recipient's Account Number" box?

Could one of you swell folks look at your AdSense 1099 and fill this info in for me? I'd be extremely grateful :)

May I gently suggest that you set up a better filing system for your business, and your personal taxes? :-)

1) Yes, the payer id is the same as what you gave.

2) Google, Inc. 2400 Bayshore Parkway, Mountain View, CA 94043 [phone]

3) Yep. There's an account number in the optional account number box. My SS is in the Recipient's identification number box.

Thank you, I appreciate you digging up that info for me. I also appreciate your concern for my filing system, though perhaps that wasn't at fault so much as our recent moving day (and Murphy's Law) resulting in a huge pile of papers from several years that will require many days to re-sort and re-file, where once existed nicely organized file boxes.

The boxes, I'm afraid, were mortally wounded, and had to be shot.
