Is there a way to determine if a particular ad is a CPM or site-targeted ad

Is there a way to tell, from looking at the ad iframe generated code, url, etc. if an ad is CPM or site-targeted? Or are there any visual cues that tell you you've been hit with one of these?
For example, if I have an AdSense ad that is 120x240, and the ad block shows only one advertiser across multiple pages, is that a sign that it's a site-targeted/CPM ad? (I assume it is.)

For that matter, are all site-targeted ads CPM ads, and vice-versa? I'm sure this is covered somewhere on Google's help site, but I couldn't find a definitive statement.
