Unlike YPN, Google had studied the map of the world and found out that people do live outside the US. They have also cottoned onto the the fact that people outise the US lead prosperous lives, and buy products from adwords advertisers. The adsense view is a global one, and they don't knock foreign visitors clicks down simply because they aren't unfortunate enough to live in the US.
Google are looking for how clicks convert. If Google devalues a click it will be because they don't feel it's going to convert for the advertiser for some reason or other.
If your site is about something of interest only to US visitors, and all the ads are targeted at the site from US advertisers, then it's likely that clicks from the rest of the world may not convert, so therefore it's sensible to smart price them.
However, if your topic is more general and likely to be of interest to a wider audience then clicks made outside the US may well have equal or better than clicks from US visitors. On my site, I find that the UK viewed page shows mostly UK ads, and the page viewed in the US shows US ads. As far as I can tell, both pay equally well.
If your site is getting low value clicks, then the reason is that Google feels the ads don't convert for advertisers, or they are showing only low value ads on your pages.
It might be an idea to browse a couple of threads in the forum library that discuss low paying clicks and smart pricing.
My sites are aimed globally for the global wholesale trade market however they also contain very relevant, essential and educational information for Joe Public purchaser.
My lowest paying clicks are from late evening, 18.00~24.00, US advertisers.
in the end, i paid several interpreters to completely word my site in the languages that covered the majority of my visitors, and because google serves ads based on content, those pages in foreign languages also now show relevant ads.

There are things you can do to raise CTR.
Firstly, your topic might be one that traditinally has low ctr - there's not always a lot you can do about it, but that being the case it may be an idea to consider cpm ads from other ad networks of you have enough traffic. You also might find that the ctr is low if it's a forum. Again, there isn't a lot you can do about this.
The main things you can do to increase ctr are to make changes in layout, and the number of ads. Have a look at the heatmap on the adsense pages. These are the positions that Google knows convert best. Also, try different tyoe of ad blocks. There can be big differences in the blocks - worth experimenting to find what works best for you. Try different ad colour schemes too - that can make a difference.
The other thing to be aware of is that putting the maximum number of ad blocks on one page absolutely kills ctr and earnings. Try just one ad block - that often increases earnings.
It's also worth saying that you need to monitor what changes have worked. No point in making loads of changes and notknowing what fixed the problem! Make changes methodically and monitor the changes that occur.
