I am in Canada, patiently waiting my check for the end of last year.. then realized I'm still getting the tax alert at the top of the stats in adsense.

I do remember going through that wizard and it not giving me anything to download and submit. I'm wondering if perhaps I made a mistake while filling it out.

to quote:
"U.S. Activities include, but are not limited to, owning a web server or owning a hosting service in the U.S., or having employees in the U.S. who are involved in either:

-setting up Publisher's web server, hosting service, or website
-developing content for Publisher's website
-marketing to create a user base for Publisher's site
-telephone support for Publisher's site
-buying products for Publisher's site
-maintaining Publisher's site"

I don't employ anyone and I used shared hosting..so wondering if I needed to fill out one of those W forms or not.

Everyone still sees that at the top of their stats - it is their little announcement for the most recent changes to the program. We will probably see that there until they have something else new and exciting to announce ;)

I'm in Canada, and in the same situation as you are -- I have not filled out any forms, and from everything I've read it isn't necessary under these circumstances. All you need to worry about is Revenue Canada in April...
