Right from the time I started with Adsense, I have followed the heat map. I have found that a 728x90 horizontal text ad unit placed below my main header in the center works best for me. There are a couple of links below my header and the ad unit comes after that.
I experimented a bit at the beginning and have used the above position since. Now I'm ready to experiment again. I would like to know what positions for ad placements have worked best for you? Are you using different layout on different pages? what size? text or image or both?
My details are:
Size: 728x90
Position: horizontally center below the main site header after a couple of link lines (making it almost center vertically too) This layout is used on all the pages that have the ads.
Ad Type: Text Only (never experimented with image ads)
Colors: No background color and border. This makes the ad blend in. Title color is rotating between 4 colors that are different from any colors used on the pages. This makes the ads stand out.
What are your details?

The other lay out on my main page has a page header the full width of the page that may have my own cpm ads in it, a nav bar left of the page and a rectangular unit (forming a drop capital in effect) putting the ad left of centre of the page. This layout works well for me, but I have changed the ad unit for an adlinks unit as they seem to be working better for me than the standard blocks currently.
