If I was banned from google adsense (2 years ago), is it possible for me to re-join under another home address, email, url? (using the same name)

It's possible to reapply under the same info as before. Whether they accept you is up to them.

Re-apply and let us know. I don't think I have heard of anyone getting turned down reapplying after two years.

you won't get further than the same e-mail. Your other problem is that you will be "living on your nerves" until you get your first cheque through. Google will get free advertising on your site for x months and you won't get a dime. Look elsewhere

Another email - ok
Another url - ok - see caveat
Using the same name - don't risk it, why not establish a trading name for yourself and the url registration?
