I recently reported a page I found stolen to AdSense, complete with my AdSense code, tracking code and everything. AdSense support shut the ads on that page down ASAP, very nice of them.
Best I can recommend is installing some AdSense tracking scripts on your pages so if they're sloppy when they steal your pages at least you'll see your ad tracking code loading via a 3rd party server and get a clue where to go looking to stop them.
Thre's no way to give a definitive answer to every scenario that could occur, but I'm sure Google is well aware of this possibility.
Easy to get around that. View/Page Source in Firefox, etc., etc.?
I have thought about this but it's not a foolproof method, people can always save the page and open it up in MS Word for example.

Why do you need to protect yourself?
Anyone anywhere at any time can get to your code. They don't have to copy it. All they have to do is find one line and paste it into their own code and they'll be able to do what you're talking about doing. You can't protect yourself from that. You can't guard against it. If you have the type of website or run with the sort of people that would require you to guard against this then don't be surprised if google bans you anway - whether you're responsible for the invalid clicks or not - just to get rid of you and not have to deal with it anymore..
There's a saying, "you're known by the company you keep."

How will PHP help you?You can put your adsense code in a .js file. ONLY allow access to that .js file if a page from within your site calls it. No one can see the code at all.

Javascript is executed in the browser. If a browser can access your scripts, then anybody who wants to have a look at them can see them.
They might need to save the page or use
Information > View JavaScript in the Web Developer extension for Firefox though, so your method may slow down the process by two or three seconds... -b
(a) It would also stop maybe 10% or so of people from seeing your ads at all. There's no reason to cut your income by that much over a minor issue like this.

That's a good point, but technically, you're not changing the code. With Google, you WILL lose that battle though.
In other areas, the competition may be much dirtier, and if you're in such an area, and being click-attacked, hacked, and so on, it's certainly possible that Google or another PPC company might not want to work with you. They don't want high-maintenance accounts.

But nothing "on-par" with worst online gambling sites.
