Yes, they do. Per Google's description:
Alternate ads count as ad unit impressions like any other ad. Google does not keep track of clicks on alternate ads, and you will not receive revenue for them.
It's within the Adsense guidelines, but I guess I can't post the link here.

I've been trying to think of an effective way to combat PSA's, and I'm not sure which alternate ads I want to use.
You'd think since the RedCross seems to be the big PSA for Google, they'd contribute $0.01 every time their ad is displayed as a PSA. That would keep publishers from using alternate ads; it would make Google money; and probably get the RedCross more hits.
if they count alternate as impressions, what's the point of coding alternate ads if other alternate networks pay less than Google, alternate URLS will only lower my CTR and eCPM
Maybe I'm missing something here, but won't alternate ads (as opposed to PSAs) increase your total revenues when AdSense ads aren't available?
EFV the income in my case from Chitika would not compensate for the lost earnings from Google with a low CTR and eCPM. I'd rather drop Google off that specific channel that does tens of thousands of daily impressions, and fix in Chitika with no Google ad code there, this way I will make the same or more from Chitika and more from Google too from my other non PSA prone channels.
