Doorway page query

On our site we have created a page for each one of our store locations. Each one has locally focused content and they perform well in local search.

Customers to these pages would certainly be interested in another service we supply, but this would involve them filling out a form on another page of our site.
If I linked to this other page from our location pages, would all of the location pages then be considered doorway pages? I currently link externally on these pages to review sites like Yelp, Yell etc, but I realise internal linking might be a problem in this case. It would be a bit of a disaster for us if these local pages got penalised, so I want to tread very carefully.

The way around it I guess is to add this form to the location page itself right?

Any thoughts on doorway pages? Has anyone been penalised for having them on your site?

This situation doesn't at all shout "doorway pages" to me. A case might be made on an extremely, extremely tight definition of doorway pages, but I still kinda' doubt it.

Do what's best for your site visitors: If "they would certainly be interested in another service" you supply, then by all mean make it easy for them to find it with links from the location pages.
