Google still indexing old pages

Google is still listing old pages from an early version of our website which was taken down a year ago.

Plus it is listing some pages where I have explicitly added <meta name="robots" content="noindex,nofollow">

Any idea how I can proactively get Google to lose my old pages and follow the new noindex,nofollow instructions?

As it happens I'm launching a website for client tomorrow and would also like to get Google to lose the old pages. Again, how do I do this?
