I should know the answer to this, but I don't.

Is there a recommended way to point to an index page in a subdirectory? Would you use the complete absolute url with the index.htm, or would you abbreviate with the directory/, without the page file name?

Would it make a difference to G in ranking or accuracy of the link?

Google doesn't care. I think this is also true of MS and Yahoo.

I always link to index.html but request that backlinks use \dir rather than index.html or \dir\
I believe the general concensus is that you should link to \dir\ being a compromise of efficiency and future-proofing (against technology changes such as moving to php).

I'd go with:

[mysite.com...]
[mysite.com...]

Or the relative-URL equivalent (no index.htm or index.html).

Apparently Google treats [mysite.com...] and [mysite.com...] as separate URLs, which can lead to duplicate listings and related problems if you get sloppy in your internal linking. Here's one of many threads from Webmaster World's Apache Web Server forum that may be helpful:
