Google Using Directory Listing Heading for SERP Title

Is this wide scale?

I've recently noticed that Google is displaying a couple of my company's websites with the website's Google directory Heading displayed instead of the physical web site page title (<title></title>)

This phenomenon has been experienced previously in the forum, but I haven't heard any recent mention of it. Is this still a very small scale occurrence?

Some have said it can only be seen on very few datacenters.

It's an absolute nightmare, especially if the DMOZ editor did a bodge job on your listing.

Must be only a small scale problem then!?

Does anyone know the cause of this problem?

If you search for something exactly how its title is listed in Dmoz then it will bring back that header.
