I instantly hit "Back" if a site displays a popup which beats my blocker, if the background is black or very dark (because I find such sites competely unreadable for my eyes), if there is a flash intro (with or without a skip function), or if it uses tiny text or no contrast. I also hit "Back" after 10 seconds absolute maximum if the page hasn't loaded.
The kids built it themselves.
It is so very tacky: every possible over-the-top design ever attempted on the web is now available in one location. If it flashes, moves, is bright and glittering, and loud it is included.
The kids are so very proud of it.
I remember my first website design. It wasn't all that different. Have I improved or just gotten old?
All of the above, but those 'designer's'full screen pop ups with everything disabled feels a bit invasive. Window resizing works every time for me.
Another thing, maybe it's just me, but the picture of a super model with a fullrack smile and a phone headset looks as phony as an info commercial, I zap them too.
A site with animated gifs
A site with those annoying sliding advertisements
A site with sound
A site with that annoying talking "person" on the site
A site that has it's main nav linking to a different site
A site with pop-ups
Any site with the "shoot the monkey" advertisement :)

Since when you need a reason to throw a party encyclo? You got married or something? :) Thanks for the welcome back.
Good to 'see' you again lawman!
How are you getting along with the new partners?

Hordes of tiny country flags linking to Babel Fish translations...
Do you still let her shop?
Equality is important in a marriage. We both have an equal right to buy clothes and shoes. ;)
Actually, she hates shopping as much as me. And she's arranged an evening out for my birthday that I wasn't supposed to know about. Ah, the joys of married life!
<yes, I know, keep it on topic>
Don't forget flowery backgrounds, or everything on the page centered.
Encyclo, web developer toolbar, disable page colors, makes the page fully readable right away, helpful when the page has actual information on it.
Almost never see any of those annoying things though, adblock takes care of almost all of it, and one come through, I just add that to my blocked list.
Flash intro, definitely, goodbye, never really see popups so wouldn't know, long lists of forum topics when I'm searching for something specific, which google would filter those out.
