Ad Links Generated-Code Problems?

I specify a 4-link, vertical Ad Links format.
I get JUST ONE, REGULAR AD?

I noticed the code supplied is different than before.
Does Google sometimes hic-cup when generating the ad code, which is obviously incorrect?

I've seen worse, if there are no ads you seem to get a search box.
