I thought of that because I have a page where a particular keyword has been mentioned in the article almost 50 times but still the adsense ad is irrelevant.

Sometimes Adsense just can not get it right, no matter what you do.

I have a few pages where I took Adsense off because the ads were NEVER on target, and so off target that it was embarassing.

But a content penalty? No. Just the limitations of the system.
