I lost AdSense after my second warning - clicked my ads the first time (categorically not to make money), got the warning, never did it again, but got the second warning a week ago - and still no July check by the way which is very odd.
Can I make a new domain, a new account, use the same content, update all my links, and get accepted into AdSense again? For me it would be a real hassle but worth the effort, the revenue transformed my site.

I would suspect you will not be able to apply for an AdSense account again, because when a site is removed from AdSense, all sites on that same account are affected, even if only 1 out of 10 sites was fraudulent - you cannot get a reprieve simply by removing the code from the one site.
I know domains are flagged to a certain extent (once a site is rejected, that domain will not show any ads, even if an active account's javascript is used). But if you were to reapply, they would probably match up names and/or addresses with those who have been rejected/removed.
You can always try contacting AdSense to see what their take on it is, but I would think their answer would also be no.
