What does that mean exactly?

For example - could you target a phrase "vioxx attorney".

Most top results are attorney sites, further down there are few sites displaying adsense - are they violating TOS, or is it just very hard to get top positions for this phrase?

I doubt you will make much with attorney terms most have figured out that the content network stinks and have dissabled it. Especially the top bidders.

While it is likely that some firms have dropped out of the network, on the whole that does not appear to be true.

But you are unlikely to make great sums targeting "last month's keyword" as opposed to anticipating "next month's".
