Something just occurred to me. I keep hearing about how much better the CTR is with AdSense ads compared to banner ads. Why don't people just set up banners to look like AdSense ads?

I think the technology behind Google's program is what keeps it a frontrunner. The reason Adsense CTRs are much higher is because they are targeted towards the content on the page. It isn't so much the "look" of an adsense ad versus a banner image, it is the content of the ad that is relevent to what you are reading about that makes it so successful.
The flexibility of the adsense program does help as well since you can practically integrate the ads into your site and many visitors won't even notice they are ads.
