I can see that there are lots of people are loosing their AdSense account for doing no wrong intentionally.
Well, not quite. Most every single person who claimed to be innocent, almost every single one, has been shown to have been breaking rules. Whether intentional or unintentional is known only to them.
If you are concerned about unintentionally breaking the rules, take a moment to familiarize yourself with them. And believe me, it is easy to unintentionally break rules.
How do you know your account won't be shut down for reasons beyond your control? How do you know that "smart pricing," a flood of other publishers in your category, advertiser turnover, or some other factor won't reduce your AdSense earnings?
IMHO, it's a lot less risky to build a site that can generate revenue in other ways (e.g., through direct ad sales or affiliate sales) so that you aren't left out on a limb if your main source of revenue--AdSense--shrinks or disappears.

But I'm still expecting some replies on the measures we can take from to not get kicked off from G.
- Read the TOS carefully and follow them strictly.
- Be vigilant of your statistics - if something isn't quite so right (e.g. income increased 10x than normal), go over your stats carefully to see if anything is amiss. And alert Google immediately.
- Monitor what others are saying about your site (get Google alert service or other services out there) so you can watch out if someone is posting something about your website in forums or saying somethings that could affect your adsense account (e.g. click on this site's Adsense ad)
People get kicked out for a reason. It's either they know what they did, or they may not be aware of what they did. Just look at the forums here and read what others are first saying ("I get kicked out for no apparent reason") only for someone else to say that that there was indeed a reason for them getting kicked out ("you posted in forums to click on your adsense ads"). There was also one complainant here who said they have a very clean site but was booted out - only to find that they do have a very clean "scraper" site. There is always a reason - your friends may simply not have been aware of it.

To add to what Alika said, the best thing you can do is have a site that will pass the "smell test."
Let's say that Google discovers invalid clicks on your site's ads. When a Google employee looks at your site, what will or she see? If the site has intrinsic value, the Google employee is more likely to think "If it looks like a rose and smells like a rose, it probably is a rose." If, on the other hand, the site has "fast-buck artist" written all over it, the publisher is less likely to get the benefit of the doubt when that busy Google employee's finger is hovering over the "KILL ACCOUNT" button.
