Signing Up for First Time

I am going to try adsense. I was reading through the FAQ and have a question.
Do you have to sign up separately for each website you want to put ads on?
Do they go and look at your website to approve it?
Any chance you will be turned down?

In fact, I was turned down when I first applied. They had decided to review my site at the same time that I was uploading revisions. They found broken links during that time and rejected the site. I emailed them back, had them take another look, and was approved.
