How do you decide where the adsense ads are placed?
When I create my site, should I create a blank table especially for where the adsense ads will be placed?

As you are suggesting, making a blank table or td from where you can put your ads into at a later point is not a bad idea. If you are creating a new site and you're going to have ads on them, you should really think about where to implement them, and not just put them where you find some blank space.
But remember, it's always smart to do a little testing on where to put the ads when your page is finished, as some pages do well having ads on the top, others should have them to the left, some should use skyscrapers and some should use blocks.. etc. etc.. So if you're smart, you code your page in a way so that you have more than 1 option on where to blend in your ads - and don't need to completely change the whole layout if you want to move them around a little ;)
Ps. Don't forget; create your site with the content and users in mind, not the money and adsense :)
