Just now I tried to add an advertiser to my filter list and was surprised that G said it was already on the list. I was surprised because I don't ever recall an advertiser showing after I had filtered it.

I normally add urls to the filter list in this format:

xyz.com

I was fooled by this advertiser because I added them as xyz.com when the domain was really:

www-xyz.com

So, now, on top everything else we have to worry about, when we're filtering ads, we must pay close attention to tricks like this.

I know, I hate these guys with the 2 "-"s, too.
Last time I saw one advertising on my site, I first reported him to google (clearly MFA with no other outbound links than AdSense) and second I searched for this advertiser in google and clicked once on his sponsored ad in the search results :)
Let them bleed!

When looking for possible MFA's to block I always do a Google search on my keywords and look at the ads that look like MFA's by clicking on them. The ones that are clearly genuine I use the preview tool for. Cost the MFA's - let 'em starve!
