Mostly per click, sometimes per impression. And no, you can't choose what you like, as a rule (nor should you, if you want to maximize income from your site).

Although strictly speaking, you can't prevent 'questionable' ads coming in, if you see them on your site you can use the competetive ad filter to stop them reappearing.

Uh-oh What do I do about the kids that visit
If you have a significant children's audience and really expect questionable ads to be a problem, then using AdSense may not be a good idea--especially since clicks by kids aren't likely to convert well for advertisers.
One more question - can I use Burst Media and Google Adsense at the same time?
Yes. See the AdSense terms and conditions and program policies.
I can see two main problems from this. When you say you get kindergarten classes on your site, kids tend to "click click click" if the class are surfing the we on the same ip address then this will make your clicks seam suspicious.
Also if your site is aimed at kids then conversions will be very low. This may end up introducing your site to smart pricing.
