Onsite Advertiser Sign Up - All Ad Units?

When you have Onsite Advertiser Sign Up turned on and more than one ad unit on a page, does the "Advertise on this site" text appear on all of the possible ad units, or just one of them?

Also, it only appears on some of the ad unit formats, correct? Can anyone list which ones?

It appears in every ad unit, any format. It's overdone and that's why I opted out of it.
