Is it against the TOS to place keywords in the alt tag of the images?

No problem whatsoever so long as the keywords are relevant to the image and the rest of the page. This also applies for titlebar and metatags too and G's algo does check that all is in "balance".

I have 1,000+ images all optimised in this manner and all ranking #1 consequently it is very important for image pages to be completed this way.

For an image and it's descriptive content to be ranked as authoritative construct an individual page for that product/article/item/whatever it may be.

Depending which country my products are from I firstly construct a general images page with about 18-21 thumbnail images all with keywords in the alt tag and all the keywords in the metatags.

I than construct one single page with a 500 x 500 image and optimise all the elements I can for that item image including as much pertinent information as possible.

However that may not necessarily be essential to rank #1 and it's useful to have something in reserve should anyone else optimise as well as oneself!

I dont think they are against TOS. Plus alt tags are useful from SEO point of view, google knows it well!
As long as it is relevant to the image and page, should be alright I believe.

What about using "title" tags along with the alt tags? Does Google consider this as content?

well title tags are very important. You should have them.
If you are new to html and not sure what meta tags to use, I suggest you seaerch for " meta tag generators" on google and you will get meta tags generated automatically for your site. Here's one www.addme.com/meta.htm

I have used it for all my sites.. saves lot of time.
