1) You're limited in how many domains you can block, and..
2) Most of the advertisers on the list would never show up on your site anyway, since AdSense ads are targeted by keyword (or occasionally by site).
I would say if you are going to block ads, then do so sparingly. If you see a MFA, then it may well be a short lived placement. I only block the persistant ones to avoid filling up the list. Also, don't rely on the preview tool - it only shows a sample set of ads. I will only block an ad if I see it on my site.
I'd dearly love Google to give us publishers more tools to block junky ads :(

I wish they didn't have any junky ads, but then, one man's poison is another man's meat.
What I'd like (but don't expect to see) would be the ability to block ads by keyword or category. For example, if I could block all booking-site ads (or all ads containing the word "hotel" or "hotels"), I might be more interested in keeping AdSense on the parts of my site that generate a lot of sales via affiliate programs. (I don't mind hotels' own ads competing with me, but I see no great benefit for me or for readers in cluttering up my pages with "PPC arbitrage" ads when I've already got affiliate links of my own.)
