I am setting up a campaign for a client, which would benefit from localising the AdWords to four regions in the UK. My previous understanding was that the IP set up in the UK makes this hard/not possible.
Any thoughts on this, from people who have tried?

Northern Ireland, Wales, England, Channel Islands, Isle of Man and Scotland are the default options for regions in the UK.
You can also type in the name of the city you wish to target if you select the 'cities' radio button.

I think your best bet is just to just set up a campaign and see how it works. Its pretty self explanatory to be honest.
As it is not a hundred percent perfect don't forget to set up another campaign targeting the whole of the UK but for keyphrases that include the location e.g. blue widgets manchester.
I think this issue should raise more concern. Google sells me a click saying that it comes from a certain zone, but if it does not I would be entilted to not pay for it.
The reality of the situation is that there will always be folks who have an IP address which effectively identifies them as being in a different place than their actual physical location. And as you can imagine, it is not possible for AdWords to verify actual location on a search-by-search basis.
If this is of real concern, then running scared was on the right track in giving the advice quoted below.

Spaidermen, you may want to consider not using regional targeting, but instead doing a 'national' campaign, and then including your location in each of your keywords. This will prevent people who are not specifying your location in their search from seeing your ads.

But please tell me what's the point of regional targeting anyway?
The point of regional targeting is to provide a simpler alternative to the more traditional method of arriving at the same result: national targeting combined with regionally specific keywords (as discussed above).
And, although it is not 100% perfect (few things are), it is certainly a reasonable alternative worth exploring.
