It's referrers, but in Apache it's spelled referer.
By the way, upon checking again, 15 to 20% of my Adwords referrers are blank for this month and last. The total number of clicks jives with the Adwords stats, so it's not just people bookmarking my tracking url.
I had a thought... if Google wanted to, they could work around the problem by offering an option to append the keywords to the end of the url that the ad points to. But it may be more work for them than it's really worth. After all, this happens sometimes with normal SERPs on other engines too.

I don't have much information to offer on how Google decides what page the ad is shown on. You might want to look into Google's AdSense program to learn more about the syndicated ads and how it works...from both a publishers standpoint and an advertisers. I currently don't use adsense or syndicate our ads so I'm not much help beyond what I have already shared. Sorry.
