Make a mourning register with ASP

Well,

my boss wants me to make a mourning register: there was a terrible accident and since we're a public service he wants something like that. So a page where people can fill in a form with their first name, last name and message and then it is either e-mailed or in the best case, put in a database and then displayed on a 'results' page, where people can read that.

- Possible scripting languages: ASP, PHP
- Possible method of collecting info: e-mail, Access database
- Method of securing the data: none

So can anyone suggest a quick & dirty way for doing such a thing?

Deadline: today... grrr...
(yes we got a programming department. No I can't bother them with it. No this isn't my task. Yes they want me to do it anyway... sigh).

get yourself a guest book script, tweek it a bit by calling it a mounring register instead of a guestbook
