i am trying to create a simple form on an asp site:

f name
l name
address
phone
email

and a check box:

how did you hear about us?
catalog or internet

Can someone please help!

What do you want the form to do (i.e., be stored to a database, be emailed to someone, etc.)? Do you need help with the HTML, the ASP, or both? Or do you have it written but are getting an error?
