Using forms in Frontpage

How to "split" them

Im trying to make a form for my webpage. I would like to know how to make it so that instead of the standard form like this

Name: txtbox

Last Name: txtbox

How do I make it so name and last name are on the same line and make it look clean?

I hope this is descriptive enough

If you still want separate fields then it's probably just as easy to put those fields in a table.

How do I put them in a table, and still have it a form?

What Im trying to do Is make a form that basically has 2 parts. Left side has name addresse etc, and the right side would have customer information.

Kinda like:

Name: How did you find us:
Last Name: How do like the site:

etc
etc

Just put a table inside the form. Then put your fields in different cells.
