Unable to get field names to save

I have created a form in FrontPage and have it set up to both e-mail the results and save the results to the server. Everything works fine, except the field names do not save on the server. I have checked the "Include field names" checkbox in the Files Results tab. The format I want them saved in is Text database using comma as a separator so I can easily import the results into Excel.

I knew I had this working a few days ago when I was testing the form as I still have those test results saved in my Excel file. I deleted formrslt.htm off the server when I was ready to go live with the form and just realized the field names are not appearing on the server now. I can't figure out what I might be doing differently!

It's not a huge problem as the form isn't that big and I can type in the field names in by hand, it's just darn annoying.

Any thoughts (other than don't use FrontPage :) )
