I'm not that familiar with doing "SEO/SEM" as an occupation, could anyone please explain the differences between them?
They both look the same to me.

SEM = Search Engine Marketing. It includes SEO and paid techniques like paid inclusion and paid placement.
SEO = search engine optimization. optimizing your site for natural listings and directories.
SEO is a piece of SEM.

Very accurately put. Optimization concentrates on the "organic" search results... concentrates on Google and then the rest... includes on the page and off the page elements... though mostly on the page.
