Yikes! Survey is too big - I expect to be paid to fill up that thing. ;)

Suggestion: The first few questions regarding username/password/contact info don't make sense. Get rid of them.

What's the research for?

Normally we charge money for research, but if it's a worthy cause we might take it. Having said that I see no reason for you to post the survey in 3 places.....

10. Keyword campaigns run constant until we discontinue a specific manufacturer, item, etc.

13. There is no limit. In our industry there are thousands of possible keywords/combinations. We bid on the most common, which are about anywhere from 500-1000 and don't limit ourselves to how many.

15. About once a month. We usually add products that often to our lines so in turn we buy keywords that coincide with that brand and/or product.
16. About once a month we fiddle around with the text of the ad or our bids.
17. Sometimes we will stop keyword buys b/c the ROI isn't there.
