Google Sitemaps Ban Remove Request Form - Success?

So I have a domain that I think Google has banned, because I violated TOS I guess. It had a PR of 5 last I checked, now it is completely gone from their database. I would like to fix the violations and have it restored of course and am patient enough to wait. Still I'm curious about people's recent success in getting Google domains unbanned.

I noticed that under Google's Sitemaps program that they now have a form specifically where you can admit guilt (looks like a traffic ticket where you admit you were speeding) and basically beg for forgiveness and ask that your domain be reinstated by Google. Has anyone actually used this? Yes, I could just try it, but I am not sure I want to grovel, and I could honestly live without this domain at this point.

I've already removed what I think were the violations. If Googlebot comes back in the future is there a chance they'll reinstate me (eventually) automatically or do I really have to ask?

I know from Las Vegas PubCon last year that Google's goal was to make re-instatement happen algorithmically. However, I don't see that this is a current reality. At conferences and in the Matt Cutts blog, I continue to hear regular suggestions from Google reps to submit a reinstatement request.
