Reinclusion requests when problem is unknown

I have recently taken over a new site. It seems to be chugging away nicely in MSN, but has issues in Google. It has it's index.html page showing on "site:domain.com" as url only, but has most other pages showing titles & descriptions.

I would submit a reinclusion request, but I really don't know why the site was penalised before. I understand that there was paid advertising and a reciprocal campaign, but nothing else specific (hidden text etc) that was a problem.

Any ideas to help me along my merry way?
