I received an order from a PO box address in Saudi Arabia. How likely is this to be fraud?

I have had a couple of orders from Saudia Arabia in the past year. They were all good. Without getting political, commiting fraud in Saudia Arabia has some harsh penalties. The P.O. Box is a red flag however. You may want to make a phone call.

Almost all mailing addresses in Saudia are PO addresses.

The ex-pats working for the oil companies live in protected compounds, usually with PO Boxes for all mail. What with the increased risk of kidnapping and car bombs, there are quite a few who won't leave the compound for the duration of their stay unless absolutely neccessary.
Having said all that, you do need to confirm this by phone - and make sure your product isn't banned in Saudi (although it's not really your problem, but having the package confiscated at customs is not ideal for anyone).
