So I was happily refactoring some code and there was this library included the purpose of which I couldn't remember so I did a quick web search... nothing, nada, no library of that name... so I went into my archive and opened it in an editor... I wrote the bl**dy thing.

As I've had this 'problem' all my life should dementia ever occur no one will notice...

One of my sites recently started throwing 500 errors during specific tasks. I thought, wow, that's weird.

I checked Apache error logs and it referenced a specific PHP file. Wow, I thought, that's really weird, why would that one file start acting up out of the blue?

Turns out I had updated the PHP file the week before but had forgotten doing it.

What happens when you're rushing!
