Looks like a handcuff for a guy with only one one arm.

But could also be some sort of clamp. Although not like any I have seen before. Some kind of medical instrument?

No, it is not a clamp. While it does have a clamping action, acting as a clamp is not its purpose.

It is a tool with a single very specific purpose.

This tool is not used for casual car maintenance. But if you rebuilt an engine, it would be surely useful.

I don't have the slightest idea what one could do with this tool in a car. I thought it looked a little bit like a tire lever.

Ok so it's used to build an engine. Special part of an engine. Plug extractor?

It is for internal engine repair.

This tool might be used if the car is oil smoking.

In the movie Cars, they were racing to win the Piston Cup. Ha ha

It looks similar to a piston ring groove cleaner.

I think I got it. It's an engine hook. You can hook a rope onto the round thing and the other end you attach to the engine and then you can use a rope pulley to lower the engine into the car!

each of your pistons (you probably have 4, 6 or 8, although engines are currently also made with 1,2,5,10&12) would typically have 3 rings - 2 for compression and one to spread and wipe oil on the cylinder walls.
the rings sit in grooves that encircle the pistons
the cleaner removes the hardened combustion byproducts that collect in the grooves.
$33 is a good price for a tool like this from snapon!
