miva merchant data collection at checkout

Howdy folks:

I'm working on a Miva Merchant store for a client, and I'm trying to do some customization. I'm an old hand at *nix and basic web design, but a new hand at Miva stuff.

The site sells clothing, and I'm trying to collect sizing information. I don't want to collect it on every product page, since it's complex sizing - seven fields.

I understand that data collection can be added during checkout, and I've seen modules in at least one place that does it, but I want to do it myself.

If anyone can point me to a tutorial (or to part of the Miva docs), I'd be greatful.

Also - if someone can recommend a reasonably priced scripting book, that would be great.

The webhost was kind enough to provide the compiler ... believe it or not.

I'm using Miva Merchant version 4.16 and I don't have OpenUI.
