I read in this forum that a Costco merchant account + Authorize.net payment gateway is the best combo. I'm talking to the Costco merchant account people at Nova right now, but who is the best Authorize.net reseller?

Nevermind. You have to be a member of Costco to use their merchant account.

I believe it is still cheaper at United Bank Service
I use the free OSCommerce and authorize.net gateway

If cost is your criteria for "best", check out deals through your bank. Authorize.net through our bank is completely free.
