Shopping Cart - No Pay

I am setting up a shopping cart for a friend of mine who owns a vet clinic. I plan on using paypal basic shopping cart for this.

This will be for items being shipped to clients (meds, etc) They also want me to set up something where people can order online but pay at the clinic and pick their order up already.

I guess I could do this with a basic form mail but I really would want it to be totaled up with tax to show the amount owed for when they come to get their order.

Can anyone provide me any direction on my best route for this?

A good place to start would be to look for shopping cart software, that supports both Paypal and email order options.
