I'm gonna be putting up an exit pop up survey as to why people did not sign up for a subscription on my site (please no debates on pop-up ethics), and on the thank you page (after the purchase).

I was just wondering what sort of questions would be useful without putting negative images into the mind of leaving surfers.
Current Exit Pop-Up Questions:
Did you think the subscription price was? fair high low
Did you find the site trustworthy? not 1-10 very
(will this put a negative image in their mind?)
The site looks professional: not 1-10 very
The site was easy to navigate: not 1-10 very
The offer didn't interest me? not 1-10 very
Does the product match your needs? Yes No- Why?
I'm planning on coming back and buying later: yes 1-10 no
I signed up for the free white paper and want to read it now: yes no

Also, on the thank you page I'd like to find out the buyers opinion of the purchase process.

Current Thank You Page Questions:
Do you think that the signup process was:
Difficulty: Easy Average Difficult
Speed: Fast Average Slow
Did you have any problems signing up? No Yes
Do you have any comments on the payment process? (text box)
Do you have any comments regarding the information you read about on the website? (text box)

Other ideas wanted!

Not as much as suggest on other question as a suggest about the order of the questions.

For the information gained to be useful, you have to look at the order of the questions and how two questions relate. for instance the answer to "The offer price was: fair, high, low" won't be that important of some answer yes to "The offer didn't interest me" except if the offer not being of interest to them was really because of price.

You want to try your survey out on a few people that you can actually physically interview before you turn it loose online. This is pretty easy to do by just having your site on a laptop and going to the mail and asking a few people to help you out for a couple of minutes in exchange for a few bucks or a gift certificate.

The popup may get rid of any future potential buyers who were just browsing, this time.

You may want to capture the 'almost sales', ie. capture the name, address, phonenumber information, as it is entered in the form fields into a database and follow up with a phone survey, based on this form data.
