How would you setup paypal to automatically pay affiliates? does it work?

I'm not sure that PayPal contains this feature, unless you are refering to Instant Payment Notification, which does allow this.
There are various help / FAQ sites which cover most of the required information to process IPN transactions, including field names, values and meanings

PayPal has a "Mass Pay" service whereby you can upload a file of affiliate payout information and disburse as a batch for amodest fee ($1 or less per payout).

I think it's only if you pay the same amount to everyone on the list.

from what I know about those type of services it is just a csv and you enter email and amount. It doesn't have to be the same amount for everyone by any means.
